Today’s health benefits landscape is more challenging than ever before. Companies of all sizes and types, from 1099 contractors up to medium size corporations are making the switch to add affordability, flexibility, and customizable health benefit options for themselves and their employees.

The FMA Freedom Select Plans are sponsored by the Small Business Agency Cooperative (SB/A CoOp).  The SB/A CoOp is a Non-Profit “Agency” Cooperative Corporation that does not buy or sell products or services but acts as the “Legal Collective Agent” of all the Cooperative Members to facilitate advantageous contractual relationships for and between the members. The SB/A CoOp may legally “aggregate” self-employed individuals (working 16 hours or more per week) and small and large employers together without becoming a Multiple Employer Welfare Association (MEWA) or acting as a Multiple Employer Trust (MET).

There are components that allow the Small Business Agency Cooperative to develop and sponsor FMA Freedom Select plans. ERISA is the Federal Law that sets the standards and regulates approximately 60% of private and public sector industry health care plans.

The Small Business Agency (SB/A) as defined above, is a specific marketplace where employers and their employees, sole proprietorships, 1099 contractors, LLC’s, and direct sales distributors can become members and access the marketplace which offers The FMA Freedom Select Plans.

The plan administrator is the bedrock and handles premiums and adjudicates claims; the prescription pharmacy, and the dental/vision are also important parts that make your healthcare plan function smoothly.

Our Management Team

David Jones

SB/A CoOp President

David Jones has over 40 years experience in the medical benefit industry and is currently the Chief Executive Officer of the Small Business/Agency Cooperative, a non-profit cooperative corporation providing employer memberships to the nonprofit cooperative.

This allows all employers and their employees to own and participate in their own self-funded ERISA Medical Benefit Program. David Jones is a co-founder of  SB/A Cooperative, Inc. where he incorporated the Cooperative in the state of Colorado as well as initiating the memberships in all 50 states.

Prior to starting the SB/A Cooperative, Mr. Jones worked with Fortune 500 companies and did resolves of health medical benefits for people throughout the years. Working through multiple A+ major insurance companies designing and marketing numerous programs, Mr. Jones brought his healthcare and insurance wisdom to today’s marketplace.

Mr. Jones’ extensive, three decades of experience in direct sales and network marketing has played a very strong part in building teams. He has lead networkers and trained others to be financially successful and to learn how to become a successful independent business owner. Mr. Jones believes that if you love what you are doing and you believe in yourself, you will become successful by helping others to do the same.

Recognizing the need of truly affordable healthcare for business employers and their staff and having the same benefit available to the marketplace, Mr. Jones set out to create the medical benefit plans known today as The SB/A Freedom Plans while combining the power of network marketing to help tens of millions of people for which health care is a dream.

Family is of utmost importance to Mr. Jones, whether it be his own or yours, therefore he has never wavered in his commitment to develop medical benefit plans for all clients. He has also designed excellent and effective marketing programs.

Mr. Jones’ personal ethics and integrity is present and employed every time he extends himself to meet and exceed his clients’ needs.

Scott Fardulis

Executive Director

Scott Fardulis is first and foremost a family man, and has been married to his amazing wife, Juliette, for over 25 years.  They have 3 children (Blake – 20, Ashton – 17, Sienna – 15), and live in beautiful Windsor, Colorado in the Highland Meadows Golf Course Community, where they enjoy a healthy lifestyle in the great outdoors.

Mr. Fardulis began his network-marketing career at the age of 17, and from his college dorm room, began building a team of students and faculty alike. He quickly learned regardless of age, education, socio-economic status or beliefs, everyone wanted the same thing … a life filled with peace, joy, and happiness.  Mentoring people in personal development, and educating them to create their life free from financial stress is his greatest passion.

Today, Scott and Juliette are Ambassadors (the highest rank in the compensation plan) with Youngevity and have been featured in a book titled, “The Greatest Networkers in the World.” Mr. Fardulis has co-founded and operated a Network Marketing Company and has consulted for several years in the home-based business field. Additionally, they have been interviewed and are featured in “The Four Year Career,” by author, Richard Bliss Brook.

With over 250,000 personal distributors doing business in 17 countries, the Fardulis couple leads Youngevity with skill set training for new and experienced network marketers.

Additionally, Scott is a seasoned business developer and entrepreneur. His experience spans 3 decades of non-profit, real-estate investing, merger & acquisition, start-up, network marketing ownership, distributor training, and personal consulting to various Chief Executives with traditional and network marketing companies.

Mr. Fardulis has a Bachelors Degree in Business Administration and attended flight school in the Bay Area of California. Most importantly, Scott enjoys being an engaged father, mentoring his kids, and pouring life’s lessons, experiences and insights into all those who come across his path and are ready to receive hope.

Mercer and Aon Sales Leadership Training; Completed the American College Registered Employee Benefit Consultant (REBC) and Registered Health Underwriter (RHU), Certified Life Underwriter (CLU), and Chartered Financial Consultant (ChFC) designation through the American College.

Frank Carrannante 

Director of Administrative Services

Frank Carrannante serves as a Board of Director of the SB/A Cooperative Inc. and is currently Director of Administrative Services and Training/Education Instructor.

Mr. Carrannante, a Licensed Insurance Producer in Life, Health, Accident since 2013, enjoys the role of Benefits Counselor while helping, educating, and supporting clients to make informed decisions about health care and employee benefits.

With a Bachelor of Science in Business Management and a distinguished aviation career for over four decades, work and fun were not always distinguishable. As an experienced entrepreneur and co-founder of a regional scheduled freight cargo airline brings valuable administrative and operational experience to SB/A CoOp Inc.

As a retired American Airlines Captain, Airline Transport Pilot, and FAA Check Airman, Mr. Carrannante possesses important skill sets in human resources, crew management, comprehensive analytical problem-solving skills, regulatory and compliance strength, and strong communication abilities.

Mr. Carrannante is a Candidate – Member of the American Society of Appraisers, ASA/USPAP Certification for Machinery and Technical Specialties.

Mr. Carrannante has two sons, Joseph – 29, and Michael – 23, both Eagle Scouts, and enjoys skiing, golf, and backpacking with them.

As a previous Scoutmaster and Award of Merit recipient with Boy Scouts of America, Mr. Carrannante believes community service is important, and guiding young adults to become leaders and responsible citizens in life, was a gratifying experience.

Sunita Sharma

Vice President & Attorney at Law

Sunita Sharma graduated from the University of Washington in Seattle, Washington in 1983 with a Bachelor of Arts in English. She received her law degree from the University of Denver College of Law in 1997. During her tenure at Denver University College of Law, Ms. Sharma served as a junior partner in the Lawyering Process Course taught at the University from 1995-1997. She was a staff member of the Preventative Law Journal and authored the article “Colorado’s Government Immunity Act – Employees Not so Immune,” published in the Spring 1997 issue.

Ms. Sharma began her career as an Assistant City Attorney for the City of Greeley, Colorado, practicing as a criminal prosecutor and also handling both state and federal civil litigation cases. She has also served as an Assistant City Attorney for Loveland, Colorado, with an emphasis in administrative law, transactional work, and representation of boards and commissions at public hearings. Ms. Sharma has also practiced in the private sector with an emphasis in civil litigation, family law, personal injury, municipal law and estate planning.

Ms. Sharma served as Municipal Judge for the Town of Gilcrest, Colorado, from 2005-2007; Assistant Municipal Judge for the City of Greeley from 2014-2017 and currently as the Deputy Municipal Judge for the Town of Berthoud. She has served as an adjunct instructor at the Institute of Business and Medical Careers. She is the former President of the Weld County Bar Association as well as the Byron White Inns of Court; She has served as a Commissioner in the 19th Judicial District Commission on Judicial Performance at the Weld County Juvenile Review Board. Ms. Sharma has additionally served as a Court Appointed Special Advocate for abused and neglected children, and as a board member of the Larimer County Bar Association. She currently serves as a Regional Coordinator of the Northern Colorado High School Mock Trial Tournament. In 2019, Ms. Sharma was appointed Chair of the Law Committee serving under the Colorado Supreme Court, as well as the representative for the state of Colorado on the National Multistate Professional Responsibilities Examination committee.

Ms. Sharma currently practices in Berthoud, Colorado, serving Boulder, Larimer, and Weld Counties. Her primary areas of practice include corporate law, personal injury, family law, and estate planning.

Dr. Jan Leslie

Chair of the Board

Dr. Jan Leslie is on the Board of Directors of the SB/A CoOp, Inc., and currently serves as Vice President, Secretary-Treasurer, and Chair. She received both her Bachelor of Science and her Doctor of Veterinary Medicine from Colorado State University, and her Master of Public Health from the University of Tennessee. She is a retired veterinarian who practiced in private practice, the military, and the government for over 20 years. She served in the U.S. Army for 18-1/2 years and served in many assignments both stateside and overseas. She was the first female commander of an MTOE (a combat ready deployable force) unit south of the Alps in Europe. Dr. Leslie has extensive experience with human resources and logistics from managing many clinics, companies, and units. She has been licensed in Health and Life Insurance since 2016. Dr. Leslie and David Jones married in 2015 and together they take great pride in their family and spending time with them.

Paul Strange

Vice President Board of Directors

Paul Strange is on the Board of Directors of the SB/A CoOp, Inc. and currently serves as a Vice President. He has been on the Finance Board of his church for the past 12 years. He served in the Army in Vietnam where he received the Bronze Star. He has been married for 46 years and has two sons and four grandchildren. Over the past 46 years he has worked in all areas of Real Estate, as a developer and investor. Paul enjoys the outdoors and mentoring others in the area of Real Estate.

Bob Young

Board of Director

Bob Young is the founder and President of BBY International Inc., a multi-state real estate development, investment and management firm founded in 1988. Bob is a graduate of the University of Colorado, married for 48 years and the father of three children and four grandchildren. While sitting on the Board of Directors of several companies, Bob also serves as Lead Elder and President of the Board of Directors of Vinelife Community Church.

Jan Fonda

Director of Print Media & Web Development

Jan Fonda is an advisory board member of the SB/A Cooperative, Inc. leading the web development and print media department as Webmaster and Graphic Artist. She has enjoyed a rewarding career in the printing and publishing industry including positions in graphic design, advertising, commercial printing, web design, magazine, and book publishing. With a lifelong interest in business and financial services, Ms. Fonda has been licensed in Life, Health, and Accident Insurance since 2013. Jan’s goal is to communicate the benefits of the SB/A Freedom Plans in a beautiful and simple way.

As a fourth-generation native, Jan has a deep love for Colorado and the West. As an artist, she pursues watercolor and acrylics, and enjoys hiking, mountain biking, skiing, kayaking, golf, and savoring our natural environment.

Juliette Fardulis

Board Advisor

Juliette Fardulis and her husband Scott have 26 years in the networking industry as field leaders, and currently hold the highest rank of Youngevity International. Juliette, a SB/A Freedom Protect Advisory Board member and a mother of three, knows first-hand that the marketplace is sorely lacking in affordable health care options. A former teacher and business journalist, Juliette’s current passion is to offer hope by promoting the SB/A Freedom Plans as the premier solution to individuals, families, and employer groups seeking cost-effective health care coverage.
